Making an Appointment:

What to bring to an appointment: Insurance & Demographic data verified at every visit.
- Insurance card(s)
- Co-payment for your insurance (If Applicable)
- Your child's and your Social Security numbers (For Insurance Purposes)
- Any changes to your Insurance, Address or Phone Number
- Name, Telephone Number and Address of your referring physician
- Referrals & authorizations from your child's primary care doctor's office (If Applicable)
- A list of your child's medications (Actual Medications are preferred for accuracy)
- A list of questions you may have for the doctor
- Books, games, snacks, formula, diapers, change of baby clothes or other necessities (Please do not bring food if your child must fast for testing.)
Preparing Your Child for an Appointment or Surgery:
To help you and your child prepare emotionally and mentally for your visit to the hospital or doctor's office, please keep the following information in mind:
- Let your child know that you feel the hospitalization, procedure or test is the right thing to do.
- Use a calm and relaxed tone of voice.
- Use honest and simple explanations.
- Let your child know in advance if you know a test or procedure is about to happen.
- Tell your child how he or she may feel, before, during and after the operation, procedure or test.
- Tell your child that friends and family will be able to visit.
- Encourage your child to ask you and the doctors and nurses lots of questions.
- Listen to your child's feelings, and help him or her talk about them.
Billing and Insurance:
Medical bills can be complicated. We want to help families understand our payment policy and their bills. Below are some frequently asked questions to help you have a better understanding of our billing and insurance policy. Are Dr. Morales and Dr. Bielefeld "in network" for my insurance?
"In network" means your insurance company has a contract with the hospital and our physicians, and usually you have smaller out-of-pocket expenses. You may choose to see an "out of network" provider, but this typically means you must pay a part or all of the bills. We encourage families to contact their insurance company directly for more information.
Do I need a referral?
Many times, yes, especially if your insurance is an HMO. You will usually need a properly completed referral for the office visit from your primary care physician. However, if the physician orders additional tests (such as an X-ray, CT scan, MRI, lab tests, physical therapy), your must have referrals for these services, too. We encourage families to contact their insurance company directly for more information.
Is the doctor and service included in the benefits my policy covers?
We do our best to understand the rules of all managed care plans, and are often able to let families know what services are covered by insurance before their visit. However, since some services provided may not be part of your benefits, we encourage families to contact their insurance company directly to find out. If services are not covered by insurance, the doctor asks for payment on the day of the visit.
